Mission Statement:
To promote comprehensive health care by providing education, information and referral services to individuals, families and professionals.
History:
Established in 1991, the Greater Adirondack Perinatal Network or GAP-Net (formerly Upper Hudson Prenatal Services Network), serves families throughout seven counties in northeastern rural upstate New York including Clinton, Essex, Franklin, Hamilton, Saratoga, Warren and Washington. GAP-Net has played an important role in assessing the region's maternal and child health needs and promoting collaboration for appropriate service development. As a result, providers can meet the needs of families in a remote rural region.
GAP-Net is one of sixteen Comprehensive Prenatal-Perinatal Service Networks, funded by the New York State Department of Health, Division of Family Health, Bureau of Women's Health. (Click here for a list of the New York State Perinatal Networks.)
The seven county area served by GAP-Net is largely rural and presents a variety of challenges in terms of health care delivery and families accessing those services.
